ご注文時にお預かりする情報と、その使いみちをまとめました。第三者への提供はご予約の提携医師のみ、インストールに必要な範囲に限ります。ご自身の情報については、いつでも開示や利用停止をご請求いただけます。What we receive when you order, what we use it for, and who else sees it. The only party we share with is the partner physician you book with, and only what your appointment needs. You can ask us at any time to see, correct or stop our use of your information.

事業者Who is responsible

  • VivoKey Japan 合同会社（代表社員 龍﨑フリオ、〒260-0015 千葉県千葉市中央区富士見一丁目14-13 千葉大栄ビル 8F）は、個人情報の保護に関する法律（以下「個人情報保護法」といいます）を遵守し、本ポリシーに従ってお客様の個人情報を取り扱います。VivoKey Japan G.K. (representative: Ryuuzaki Julio), 8F Chiba Daiei Building, 1-14-13 Fujimi, Chuo-ku, Chiba-shi, Chiba 260-0015, complies with the Act on the Protection of Personal Information and handles your information under this policy.

個人情報とはWhat counts as personal information

  • 個人情報保護法第2条第1項に定める、生存する個人を識別できる情報（他の情報と容易に照合して識別できるものを含みます）です。当社では主に、ご注文時にお預かりする氏名、ご連絡先、ご注文内容がこれにあたります。Information about a living individual that can identify that person, directly or by easy combination with other information, as defined in Article 2, Paragraph 1 of the Act. For us this is mainly the name, contact details and order details we receive when you order.
  • マイナンバーや旅券番号などの個人識別符号が含まれる情報も、個人情報にあたります。Personal identifier codes, such as an individual number (My Number) or a passport number, also count as personal information.

利用目的What we use it for

  • サービスの提供のため。お問い合わせへの対応、ご注文のご案内、インストールバウチャーの発行、提携医師のもとへの製品の発送、発送した製品とお客様の照合。To provide the service: answering enquiries, guiding your order, issuing the installation voucher, shipping the implant to the partner physician, and matching you to the shipped unit.
  • ご予約の提携医師との調整、サポートの提供、規約等の変更のお知らせ、規約等に違反する行為への対応のため。To coordinate your appointment with the partner physician, to provide support, to notify you of changes to our terms, and to respond to any breach of them.
  • 当社の製品やサービスのご案内のため、および上記に付随する目的のため。To tell you about our products and services, and for purposes incidental to the ones above.
  • サービスの改善や新しいサービスの開発のため（個人を識別できない形に加工した統計を含みます）。利用目的は、関連性が合理的に認められる範囲で変更することがあり、その場合はお知らせします。To improve the service and develop new services, including statistics prepared in a form that cannot identify anyone. We may change these purposes only where the new purpose is reasonably related to the original one, and we will announce any change.

提携医師への提供What the partner physician receives

  • ご予約の提携医師には、氏名とご注文内容を提供します。ご連絡先は、ご予約に必要な場合にかぎり提供します。この提供については、ご注文時の確認事項でご同意いただきます。The partner physician you book with receives your name and order details. Your contact details are shared only if the appointment needs them. You agree to this when you order, on the acknowledgements page.
  • 提携医師は、医療上の記録を独自に管理します。そこでの情報の取り扱いは、当社ではなく提携医師が行います。The partner physician keeps their own medical records. How they handle your information there is their responsibility, not VivoKey Japan's.

第三者提供Other third parties

  • 上記以外に、法令で認められる場合（法令にもとづくとき、人の生命・身体・財産の保護に必要で同意を得ることが難しいとき、公衆衛生の向上や児童の健全な育成のために特に必要なとき、公的機関が法令の定める事務を行うことへの協力が必要なときなど）を除き、あらかじめ同意を得ずに個人情報を第三者に提供したり、利用目的の範囲を超えて取り扱ったりしません。Beyond that, we do not provide personal information to third parties, or use it beyond the stated purposes, without your prior consent. The law allows exceptions: where a law requires it, to protect life, body or property when consent is hard to obtain, where it is especially necessary for public health or for the healthy development of children and consent is hard to obtain, or to cooperate with a public authority carrying out its legal duties.
  • 業務の一部を委託する場合、事業承継に伴う提供、個人情報保護法の定めにもとづく共同利用は、第三者提供にあたりません。委託先には、個人情報が安全に管理されるよう必要な監督を行います。We do not treat the following as providing your information to a third party: entrusting part of the work to a contractor, a transfer that comes with a business succession, or joint use as the Act provides for. Contractors are supervised so that your information stays protected.

安全管理How it is protected

  • 個人情報は偽りその他不正の手段によらず適正に取得し、利用目的の範囲内で扱います。紛失、破壊、改ざん、漏えいを防ぐため、必要な人だけがアクセスできる体制や従業員への必要な監督など、組織的・技術的な安全管理措置を講じます。We obtain personal information fairly, never by deception or other improper means, and use it only within the stated purposes. Organizational and technical measures guard against loss, destruction, alteration and leakage, including access limited to the people who need it and the necessary supervision of staff.

開示・訂正・利用停止Disclosure, correction and suspension

  • ご自身の個人情報について、開示、訂正、追加、削除、利用停止をご請求いただけます。ご本人からのご請求であることを確認のうえ、法の定めに従い、遅滞なく対応します。該当する情報がない場合や、訂正等を行わないと決めた場合も、その旨をお知らせします。You may ask us to disclose, correct, add to, delete, or stop using your personal information. After confirming the request comes from you, we respond without undue delay, as the Act requires. If we hold no such information, or decide not to make a change, we tell you so.

  • 本サイトではCookieを使用していません。アクセス解析も行っていません。表示言語とテーマの設定のみ、お使いのブラウザ内（ローカルストレージ）に保存します。この設定は当社に送信されません。ブラウザの設定で消去できます。This site sets no cookies and runs no analytics. Your language and theme choices are stored in your own browser (local storage) and are never sent to us. You can clear them in your browser settings.
